The Child, Youth, and Family Program at the Transition House provides specialized wrap-around support to mothers and their children who are staying at the SOWINS Transition House. The Family Support Worker and the Child & Youth Support Worker provide mothers with parenting support & education, accompaniment to appointments, safety assessment and planning, advocacy, and other stabilization supports including securing housing and referrals to other agencies.
Our team works collaboratively with families to identify their physical, emotional, and legal needs and assist them in achieving their goals during their stay. Referrals to SOWINS and other agency outreach services are made to provide ongoing support services to women and children from the time of their departure from the Transition House until they establish independence in the community.
The Child, Youth, and Family Program team strives to build healthy relationships with children and youth through therapeutic crafts and trauma-informed activities that are sensitive to the losses they have suffered and the abuse that they may have experienced.
